Flu Shots
Flu shots should be performed at a Doctor’s Office to be covered with no extra paperwork or cost to you. If given at a drug store (ex: Walgreens, Rite Aid), you will have to pay the cost upfront, then use the attached form along with the required paperwork in order to get your reimbursement from Blue Cross Blue Shield.
